Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
- Conduct 60-minute evaluations and 30-minute follow-up sessions in an outpatient setting
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned
- Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
Required Qualifications- Active and unrestricted Physical Therapist licensure in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
- Knowledge of and adherence to discipline-specific professional code of conduct and state practice act
- Demonstrated commitment to patient safety and safe patient handling practices
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience with clinical student supervision and mentorship
- Familiarity with performance improvement methodologies and quality initiatives
