Key Responsibilities
  • Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
  • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Design functional treatment programs that support effective return to community activities, progressing patients toward discharge within an established length of stay
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
  • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements while actively pursuing continuing education and professional development
Required Qualifications
  • Active and unrestricted Physical Therapist license in good standing in the state of Colorado
  • Ability to perform 60-minute evaluations and 30-minute follow-up visits in an outpatient setting
  • Proficiency in clinical reasoning, treatment planning, and goal writing in accordance with state and federal guidelines
  • Commitment to patient and employee safety standards, including safe patient handling and infection control practices
  • Completion of all state-mandated continuing education requirements and adherence to the discipline-specific professional code of conduct
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring physical therapy students
  • Familiarity with performance improvement methodologies and quality initiative participation
