Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care teams to support patient outcomes
- Educate patients and families on therapeutic exercises and home programs
Required Qualifications
- Active Physical Therapist (PT) license in the state of New Jersey
- Graduate of an accredited Physical Therapy program
- Experience working in a SNF or similar post-acute care environment
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior travel or contract therapy experience
- Familiarity with skilled nursing documentation and Medicare guidelines
