Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
- Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
- Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management including coverage acceptance and plan of care communication.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Actively participate in professional development, continuing education, performance improvement initiatives, and patient and professional advocacy efforts.
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the state of Wisconsin.
- Adherence to the discipline-specific State Practice Act and all applicable regulatory agency requirements.
- Completion of all state-mandated continuing education requirements.
- Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Ability to maintain compliance with state, federal, and intermediary documentation and billing guidelines.
Preferred Experience / Skills- Experience in an inpatient rehabilitation or acute care hospital setting.
- Experience with student clinical education, including supervision and evaluation of therapy students.
- Familiarity with interdisciplinary team collaboration and discharge planning processes.
