Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
  • Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
  • Effectively manage individual, concurrent, and group treatment sessions based on patient care needs, demonstrating flexibility in all aspects of caseload management.
  • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
  • Actively participate in professional development, continuing education, and performance improvement initiatives in alignment with organizational objectives.
Required Qualifications
  • Current, active Physical Therapist licensure in good standing in the state of Georgia.
  • Adherence to the state-specific Physical Therapy Practice Act and all applicable regulatory agency requirements.
  • Completion of all state-mandated continuing education requirements.
  • Demonstrated ability to perform comprehensive patient evaluations and develop clinically sound treatment plans.
  • Knowledge of documentation requirements in compliance with state, federal, and insurance guidelines.
Preferred Experience / Skills
  • Experience working in an inpatient rehabilitation or acute care hospital setting.
  • Familiarity with interdisciplinary team collaboration and discharge planning processes.
  • Prior experience supervising or mentoring Physical Therapy students.
