Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities.
- Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education.
- Support organizational performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels.
Required Qualifications- Active and unrestricted Physical Therapist licensure in the state of California.
- Ability to complete comprehensive patient evaluations and develop clinically reasoned treatment plans with discharge-focused goals.
- Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
- Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards.
- Commitment to patient and employee safety practices including safe patient handling, infection control, and hazard reporting.
Preferred Experience / Skills- Experience supervising or mentoring Physical Therapy students in a clinical setting.
- Active involvement in professional development activities, continuing education, or professional organizations.
- Familiarity with performance improvement methodologies and quality improvement initiatives in a clinical environment.
