Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Conduct 60-minute evaluations and 30-minute follow-up sessions; progress patients toward discharge within an established length of stay
  • Complete all medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
  • Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific State Practice Act and regulatory requirements; actively pursue continuing education and professional development
Required Qualifications
  • Active and unrestricted Physical Therapist license in good standing in the state of Colorado
  • Experience conducting outpatient physical therapy evaluations and treatments
  • Proficiency in medical record documentation in compliance with state and federal guidelines
  • Knowledge of and adherence to discipline-specific professional code of conduct and State Practice Act
  • Completion of all state-mandated continuing education requirements
Preferred Experience / Skills
  • Experience supervising or mentoring physical therapy students in a clinical setting
  • Familiarity with performance improvement methodologies and quality improvement initiatives
  • Experience with functional quality indicator assessments and letters of medical necessity
