Physical Therapist – Outpatient

Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Ensure care is provided under signed, valid physician orders and within insurance approvals as required for patient clinical care
- Delegate and direct discipline-specific therapy services for assigned patients, designing functional treatment programs to support effective return to community activities
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
- Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the applicable state
- Minimum of 1 year of clinical experience as a Physical Therapist
- Adherence to discipline-specific State Practice Act and all applicable regulatory agency requirements
- Completion of all state-mandated continuing education requirements
- Demonstrated ability to perform in an ethical manner in accordance with the discipline-specific professional code of conduct
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Familiarity with performance improvement methodologies and quality of care initiatives
- Experience supervising or mentoring physical therapy students
