Shift: 8:00 AM – 4:30 PM
Schedule: Sunday through Saturday
Guaranteed Hours: 35 hours per week
We are seeking a dedicated and experienced Physical Therapist (PT) to join our team at a long-term care facility in Leeds, MA. In this role, you will assess residents and create individualized treatment plans aimed at improving mobility, reducing pain, and enhancing overall quality of life.
Responsibilities:Evaluate patients and develop customized treatment plans
Deliver therapeutic exercises and manual therapy techniques
Monitor patient progress and adjust care plans as needed
Maintain accurate and timely documentation
Educate patients, families, and caregivers on therapy programs
Collaborate with an interdisciplinary healthcare team
Active Physical Therapist license in the state of Massachusetts
Prior experience in long-term care preferred
Strong clinical reasoning and communication skills
Ability to work independently and manage a caseload
