Key Responsibilities- Complete comprehensive evaluations for assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Perform 60-minute initial evaluations and 30-minute follow-up appointments in an outpatient setting
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support return to community activities
- Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned, including supervision, development of learning objectives, and completion of student evaluations
- Adhere to discipline-specific state practice acts and regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Colorado
- Experience performing comprehensive patient evaluations and developing individualized treatment plans
- Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
- Commitment to patient and employee safety standards and safe patient handling practices
- Ability to adhere to professional codes of conduct and discipline-specific ethical standards
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Demonstrated involvement in continuing education and professional development activities
