Contract | Full-Time | Onsite | Days | 40 hrs/week | Monday–Wednesday 10:30am–7:00pm, Tuesday–Thursday 7:00am–3:30pm, Friday 9:00am–5:30pm

• Provide outpatient physical therapy services to adults, adolescents, and geriatric patients
• Manage an average caseload of approximately 55 visits per week
• Conduct fall risk assessments and gross motor assessments
• Screen patients and build caseload within the outpatient setting
• Treat a broad range of orthopedic conditions including post-operative care, total joint replacement, rotator cuff repair, disc herniation, ACL tears, and more
• Document patient care and progress using NextGen EMR

• Active Physical Therapist license in the state of Illinois
• Minimum of 1 year of outpatient physical therapy experience
• Experience treating adults, adolescents, and geriatric patient populations
• Proficiency or ability to work with NextGen EMR
• Business casual dress code compliance required

• Experience with a wide variety of orthopedic diagnoses including plantar fasciitis, SI joint dysfunction, stenosis, and postural dysfunction
• Experience performing fall risk and gross motor assessments in an outpatient setting
