Key Responsibilities- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver direct patient care focused on improving mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
- Educate patients and their families on therapeutic exercises and home care strategies
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Washington
- Graduate of an accredited Physical Therapy program
- Current BLS/CPR certification
- Experience working in a skilled nursing facility (SNF) setting
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with MDS assessments and skilled nursing documentation requirements
