Physical Therapist – Home Health (Travel Contract)

Key Responsibilities- Provide physical therapy services to patients in a home health setting across an assigned geographic territory on the island of Oahu
- Complete a minimum of 25 patient visits per week to meet guaranteed hours requirements
- Conduct and complete OASIS assessments accurately and in a timely manner
- Maintain timely clinical documentation in compliance with agency and regulatory standards
- Travel independently between patient homes within the assigned coverage territory using personal transportation
- Participate in on-call rotation as required and complete all required orientation modules
Required Qualifications- Active Hawaii state Physical Therapist license (pending license with eTA accepted if applied upon submission)
- Minimum of 1 year of Home Health experience
- OASIS experience required
- BLS certification through the American Heart Association (AHA) required
- Active, valid driver's license and access to personal transportation
- Commitment to timely communication and clinical documentation
Preferred Experience / Skills- OASIS experience strongly preferred
- Candidates without Home Health experience will be considered if they hold outpatient physical therapy experience
- Familiarity with home health compliance standards and field-based care delivery
