Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Perform 60-minute evaluations and treatments; maintain clinical reasoning focused on progressing patients toward discharge within an established length of stay
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations
- Adhere to discipline-specific State Practice Act and all regulatory requirements; uphold professional standards and codes of conduct
Required Qualifications- Active and unrestricted Physical Therapist licensure in the state of Ohio
- Graduate of an accredited Physical Therapy program
- Ability to complete comprehensive patient evaluations and develop appropriate, goal-oriented treatment plans
- Proficiency in medical record documentation in compliance with state and federal guidelines
- Commitment to patient and employee safety standards including safe patient handling and infection control practices
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring Physical Therapy students
- Active involvement in continuing education and professional development activities
