Key Responsibilities
Provide physical therapy evaluation and treatment services within a skilled nursing facility (SNF) setting
Deliver patient care across assigned worksites, with openness to floating between locations as needed
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to support patient rehabilitation goals
Adhere to all credentialing, compliance, and licensure requirements throughout the assignment
Required Qualifications
Active Physical Therapist license in the state of California
Minimum of 1 year of experience in a skilled nursing facility (SNF) setting
Willingness to float between worksites as operationally required
Ability to work 8-hour day shifts, 5 days per week
Preferred Experience / Skills
Experience using Casamba clinical documentation software
Familiarity with SNF workflows, productivity standards, and Medicare documentation requirements
