• Provide direct physical therapy services to patients in an outpatient therapy setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs
• Implement therapeutic interventions and monitor patient progress toward goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with the care team to ensure coordinated and effective patient outcomes

• Active Physical Therapist license in the state of Indiana (permanent license required)
• Prior experience in an outpatient or relevant physical therapy setting
• Ability to meet facility-specific credentialing and documentation requirements prior to assignment start
• Candidates who have worked in a permanent or PRN capacity at this facility within the last year are not eligible for this assignment

• Experience working in a high-volume outpatient therapy environment
• Familiarity with travel or contract therapy assignments

• Equipment and parking details provided upon start, ensuring a smooth onboarding experience
• Consistent, full-time contract schedule with weekly billing and guaranteed hours
