Schedule / Job TypeContract | 13-Week Assignment | Skilled Nursing Facility (SNF)
Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Document patient progress, treatment outcomes, and clinical notes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
- Educate patients and caregivers on therapeutic exercises and home programs to support continued recovery
Required Qualifications- Active Physical Therapist (PT) license in the state of Oregon or ability to obtain prior to start
- Graduate of an accredited Physical Therapy program
- Current BLS/CPR certification
- Prior experience working in a skilled nursing or long-term care setting
Preferred Experience / Skills- Experience with MDS assessments and PDPM documentation
- Familiarity with electronic medical records (EMR) systems used in SNF environments
