Key Responsibilities- Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Supervise and mentor students as assigned, providing a positive learning environment with appropriate oversight, learning objectives, and completion of student evaluations
- Adhere to discipline-specific State Practice Act and all regulatory requirements, maintaining professional licensure in good standing
- Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision to maximize outcomes
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Colorado
- Ability to perform 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting
- Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
- Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
- Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills- Experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Active involvement in continuing education and professional development activities
