Key Responsibilities
  • Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Supervise and mentor students as assigned, providing a positive learning environment with appropriate oversight, learning objectives, and completion of student evaluations
  • Adhere to discipline-specific State Practice Act and all regulatory requirements, maintaining professional licensure in good standing
  • Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision to maximize outcomes
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Colorado
  • Ability to perform 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting
  • Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
  • Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
  • Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills
  • Experience in an outpatient physical therapy setting
  • Experience supervising or mentoring physical therapy students
  • Active involvement in continuing education and professional development activities
