    Key Responsibilities
    • Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
    • Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation, modalities, use of supportive and assistive devices, and wound care
    • Assess and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes for effectiveness in the electronic documentation system
    • Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders and discharge planning
    • Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
    • Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed
    Required Qualifications
    • Active Massachusetts Physical Therapist state license
    • Minimum of 1 year of physical therapy experience, or completion of at least a Level II clinical experience (new graduates considered)
    • Experience in long-term care or rehabilitation settings
    • Adherence to APTA and Massachusetts state practice acts, code of conduct, and professional ethics
    Preferred Experience / Skills
    • Experience working with Medicare A, Medicare B, and managed care patient populations
    • Familiarity with electronic documentation systems used in long-term care or rehabilitation environments
    • Experience conducting evaluations and managing a varied patient caseload
