Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
  • Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
  • Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management and ensuring the plan of care is communicated and progressed throughout the length of stay.
  • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
  • Actively participate in performance improvement initiatives, professional development activities, and patient and professional advocacy efforts.
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Florida.
  • Adherence to the state Physical Therapy Practice Act and all applicable regulatory agency requirements.
  • Completion of all state-mandated continuing education requirements to maintain licensure in good standing.
  • Demonstrated ability to perform comprehensive patient evaluations and develop clinically appropriate treatment plans.
  • Ability to complete thorough and timely medical record documentation in compliance with applicable guidelines and policies.
Preferred Experience / Skills
  • Experience working in an inpatient rehabilitation or acute care hospital setting.
  • Familiarity with interdisciplinary team collaboration and discharge planning processes.
  • Experience supervising or mentoring Physical Therapy students.
