Key Responsibilities- Provide direct physical therapy services to patients in accordance with physician orders and established care plans
- Evaluate patient functional abilities and limitations to develop individualized treatment plans
- Implement therapeutic interventions to improve patient mobility, strength, and overall physical function
- Document patient progress, treatment outcomes, and any changes in condition in a timely and accurate manner
- Collaborate with interdisciplinary care team members to ensure coordinated and effective patient care
Required Qualifications- Active Physical Therapist (PT) license in the state of Iowa or eligibility for licensure
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Ability to commit to guaranteed hours: 36 hours per week for twelve-hour shifts, or 40 hours per week for eight to ten hour shifts
Preferred Experience / Skills- Prior experience in a skilled nursing or long-term care setting
- Strong documentation and communication skills
