Contract | Full-Time (40 hours/week) | Days | Outpatient | 6-Month Contract

Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Conduct 60-minute evaluations and 30-minute follow-up appointments within an outpatient setting, split across two clinic locations
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Design functional treatment programs that support effective patient return to community activities and progress patients toward discharge within an established length of stay
  • Delegate and direct discipline-specific therapy services for assigned patients while adhering to signed, valid physician orders and insurance approvals
  • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
Required Qualifications
  • Active and unrestricted Physical Therapist licensure in the state of Colorado
  • Adherence to the Colorado Physical Therapy Practice Act and all applicable regulatory agency requirements
  • Completion of all state-mandated continuing education requirements
  • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
  • Commitment to patient and employee safety standards, including safe patient handling and infection control practices
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience with clinical documentation systems and functional quality indicator assessments
  • Experience supervising or mentoring physical therapy students
