Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Conduct 60-minute initial evaluations and 30-minute follow-up appointments in an outpatient setting across two clinic locations
  • Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation
  • Adhere to the applicable State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill continuing education obligations
Required Qualifications
  • Active and unrestricted Physical Therapist licensure in the state of Colorado
  • Graduate of an accredited Physical Therapy program
  • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
  • Proficiency in medical record documentation in compliance with state, federal, and insurance guidelines
  • Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring Physical Therapy students
  • Demonstrated involvement in continuing education and professional development activities
