Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations and 30-minute follow-up appointments in an outpatient setting across two clinic locations
- Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation
- Adhere to the applicable State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill continuing education obligations
Required Qualifications- Active and unrestricted Physical Therapist licensure in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
- Proficiency in medical record documentation in compliance with state, federal, and insurance guidelines
- Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring Physical Therapy students
- Demonstrated involvement in continuing education and professional development activities
