• Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Complete thorough medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Ensure all care is provided under signed, valid physician orders and within insurance approvals as required
• Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
• Contribute to organizational performance improvement initiatives and apply continuing education to meet strategic objectives

• Active and unrestricted Physical Therapist license in good standing in the state of Minnesota
• Adherence to the state Physical Therapy Practice Act and all applicable regulatory requirements
• Completion of all state-mandated continuing education requirements
• Ability to perform comprehensive patient evaluations and develop clinical treatment plans using sound clinical reasoning
• Commitment to ethical practice in accordance with discipline-specific professional codes of conduct

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality initiatives in a clinical environment
