Key Responsibilities- Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Complete thorough medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Ensure all care is provided under signed, valid physician orders and within insurance approvals as required
- Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
- Contribute to organizational performance improvement initiatives and apply continuing education to meet strategic objectives
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the state of Minnesota
- Adherence to the state Physical Therapy Practice Act and all applicable regulatory requirements
- Completion of all state-mandated continuing education requirements
- Ability to perform comprehensive patient evaluations and develop clinical treatment plans using sound clinical reasoning
- Commitment to ethical practice in accordance with discipline-specific professional codes of conduct
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Familiarity with performance improvement methodologies and quality initiatives in a clinical environment
