Key Responsibilities
  • Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific state practice acts and all applicable regulatory requirements, maintaining professional licensure in good standing
  • Participate in performance improvement initiatives and contribute feedback to maximize quality of care outcomes
Required Qualifications
  • Active Physical Therapist license in good standing in the state of Colorado
  • Experience conducting outpatient physical therapy evaluations (60-minute evals) and follow-up visits (30-minute intervals)
  • Ability to develop and implement individualized treatment plans aligned with physician orders and insurance approvals
  • Knowledge of and adherence to state practice acts, professional codes of conduct, and applicable regulatory standards
  • Completion of all state-mandated continuing education requirements
Preferred Experience / Skills
  • Experience in an outpatient rehabilitation or clinic setting
  • Demonstrated commitment to ongoing professional development through continuing education and professional organization involvement
  • Familiarity with performance improvement methodologies and quality indicator documentation
