Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
- Conduct 60-minute evaluations and 30-minute follow-up appointments, ensuring progression toward discharge within an established length of stay
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals
- Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
- Adhere to state Practice Act and regulatory requirements, maintain professional licensure in good standing, and actively pursue continuing education and professional development
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the state of Colorado
- Experience performing comprehensive patient evaluations and developing evidence-based treatment plans
- Proficiency in medical record documentation in compliance with state and federal guidelines
- Knowledge of and adherence to discipline-specific state Practice Act and professional code of conduct
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Familiarity with performance improvement methodologies and quality initiatives
