Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing.
- Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations.
Required Qualifications- Active and unrestricted Physical Therapist licensure in the state of Connecticut.
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Proficiency in clinical documentation in compliance with state, federal, and payer guidelines.
- Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct.
- Commitment to patient and employee safety standards and protocols.
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting.
- Experience supervising or mentoring physical therapy students.
- Familiarity with performance improvement methodologies and quality initiatives.
