Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
- Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
- Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in all aspects of caseload management.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
- Participate in the student program as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision to maximize quality outcomes.
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the state of Kentucky.
- Adherence to the state Physical Therapy Practice Act and all applicable regulatory agency requirements.
- Completion of all state-mandated continuing education requirements.
- Demonstrated competency in inpatient rehabilitation patient care, including safe patient handling and materials handling practices.
- Ability to complete timely and accurate medical record documentation in compliance with hospital policies and state and federal guidelines.
Preferred Experience / Skills- Prior experience in an inpatient rehabilitation setting.
- Experience supervising or mentoring Physical Therapy students.
- Active involvement in professional organizations or continuing education beyond minimum state requirements.
