Physical Therapist – Outpatient Ortho/Sports

Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
  • Maintain clear, concise, accurate, and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to the applicable State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
  • Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision to maximize quality outcomes
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Arizona
  • Experience in outpatient orthopedic and/or sports medicine settings
  • Ability to manage a caseload of approximately 10–11 visits per day in an 8-hour shift
  • Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
  • Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
  • Experience treating a diverse age population across orthopedic and sports rehabilitation diagnoses
  • Demonstrated involvement in continuing education, professional organizations, or self-directed professional development
  • Familiarity with performance improvement methodologies and quality initiative participation
