Key Responsibilities
  • Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Conducts 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting
  • Completes accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Delegates and directs discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Adheres to the applicable State Practice Act and all regulatory requirements; maintains professional licensure in good standing and fulfills continuing education requirements
  • Serves as an educator for patients, families, students, and coworkers; participates in student programs as assigned, providing appropriate supervision and completing student evaluations
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Colorado
  • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
  • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
  • Knowledge of and adherence to discipline-specific professional code of conduct and applicable State Practice Act
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring Physical Therapy students
  • Demonstrated commitment to continuing education and ongoing professional development
