Schedule / Job TypeContract | Days | Outpatient | On-Site (split between two locations)
Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations and 30-minute follow-up appointments in an outpatient setting
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
- Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
- Adhere to discipline-specific State Practice Act and all regulatory agency requirements while maintaining professional licensure in good standing
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Colorado
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
- Proficiency in medical record documentation compliant with state, federal, and payer guidelines
- Knowledge of and adherence to discipline-specific professional code of conduct and State Practice Act
- Commitment to patient and employee safety standards and protocols
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring PT students in a clinical environment
- Familiarity with performance improvement methodologies and quality initiatives
