• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Conduct 60-minute initial evaluations and 30-minute follow-up visits in an outpatient setting
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
• Delegate and direct discipline-specific therapy services for assigned patients
• Serve as an educator for patients, families, students, and coworkers, and participate in student programs as assigned

• Active and unrestricted Physical Therapist licensure in good standing in the state of Colorado
• Adherence to state-specific Physical Therapy Practice Act and all applicable regulatory agency requirements
• Ability to ensure care is provided under signed, valid physician orders and within insurance approvals
• Completion of all state-mandated continuing education requirements
• Demonstrated commitment to patient and employee safety standards

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality of care initiatives

• Work in a structured outpatient environment with defined evaluation and follow-up scheduling that supports quality patient care
