Key Responsibilities- Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
- Adhere to the state Physical Therapy Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
- Participate in performance improvement initiatives and contribute feedback to maximize quality of care outcomes
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Colorado
- Experience performing 60-minute evaluations and 30-minute follow-up visits in an outpatient setting
- Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
- Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Demonstrated commitment to ongoing professional development through continuing education and professional organization involvement
