Key Responsibilities- Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities within an established length of stay
- Delegate and direct discipline-specific therapy services for assigned patients
- Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
- Adhere to discipline-specific State Practice Act and regulatory requirements; actively pursue professional development through continuing education and professional organizations
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the state of Colorado
- Ability to perform 60-minute evaluations and 30-minute follow-up treatment sessions in an outpatient setting
- Proficiency in clinical documentation including goal writing, medical necessity letters, and charge entry in compliance with applicable guidelines
- Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts
- Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Familiarity with performance improvement methodologies and quality of care initiatives
