Physical Therapist (PT) - Home Health

Schedule / Job TypeContract | Full-Time | Onsite | Monday–Friday, 8:00 AM – 5:00 PM | 13-Week Assignment
Key Responsibilities- Provide skilled physical therapy services to patients within a home health setting across the Anchorage municipality territory
- Conduct patient evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the home environment
- Complete OASIS assessments accurately and in accordance with regulatory requirements
- Maintain productivity expectations within a 40-hour work week
- Document all patient care activities and progress in the electronic medical records system in a timely manner
Required Qualifications- Active Physical Therapist (PT) license valid in the state of Alaska
- Minimum of 1 year of home health experience
- Minimum of 1 year of OASIS experience
- Ability to work a Monday–Friday daytime schedule with no on-call or weekend requirements
- Reliable transportation to travel throughout the Anchorage municipality territory
Preferred Experience / Skills- Experience with HCHB (Homecare Homebase) electronic medical records system
- Familiarity with home health productivity standards and point-based documentation systems
