Description: This position primarily involves outpatient physical therapy. The role may occasionally require floating to acute care settings. Responsibilities include evaluating patient functions through formal and informal testing measures. Developing treatment plans based on assessment data and patient tolerance is essential. The treatment plans must be authorized and coordinated with the physician. Updating the plan of care as needed to reflect changes in patient status is required. Developing a specific schedule of treatment and discussing it with patients and caregivers is necessary. Education of patients, families, and caregivers regarding conditions and treatment rationales is key. Supervision of Physical Therapist Assistants and aides is part of the role. Recommendations for adaptive and supportive equipment based on assessments are needed. Documentation of patient responses to treatment and changes in conditions must comply with policies. Daily documentation for billing procedures must be completed accurately. Providing treatment interventions to facilitate or restore optimal functioning is expected. Education of patients and caregivers in home education programs is required. Effective communication with internal and external customers to maximize quality patient care is critical. Participation in program development and interdisciplinary meetings is encouraged. Promoting physical therapy services within the community is also part of the responsibilities. A commitment to service excellence, integrity, and teamwork is essential. Maintaining confidentiality of patient information in accordance with policies is mandatory. Other duties may be assigned as needed. This position may involve exposure to stressful situations and potentially infectious materials. Employees must demonstrate the ability to perform essential functions while maintaining safety protocols. Maintaining regular attendance and completing all tasks accurately is required. Physical demands include the ability to lift and carry weight as necessary for treatment and care. Overall dexterity is required for various physical therapy tasks.
