Key Responsibilities
- Provide physical therapy services to patients in their homes in accordance with physician orders and care plans
- Perform patient evaluations and start-of-care visits
- Develop and implement individualized therapy treatment plans
- Educate patients and caregivers on exercises, mobility techniques, and safety strategies
- Document patient progress and treatment outcomes in the EMR system
- Collaborate with nurses, physicians, and interdisciplinary care teams
- Complete required clinical documentation including OASIS assessments when applicable
Clinical Support & Workflow
- PTs focus primarily on evaluations and start-of-care visits
- Physical Therapist Assistants (PTAs) assist with follow-up visits
- Dedicated scheduling team manages patient visit scheduling
- EMR system: Homecare Homebase (HCHB)
