Pride Health is hiring a Physical Therapist to support our clients in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!

Job Title -Physical Therapist Home Health

Job Type: Contract

Location- Edensburg, PA

Shift:  Monday–Friday, 

32 hours/ week 

Pay -$54 to $56/ hr on W2 

Coverage Areas

  • Cambria County
  • Northern Cambria 
  • Indiana County

Key Responsibilities:

  • Perform comprehensive patient evaluations and develop individualized plans of care
  • Provide skilled physical therapy services in patients’ homes
  • Conduct Start of Care (SOC), Resumption of Care (ROC), and discharge visits
  • Monitor patient progress and modify treatment plans as needed
  • Educate patients and caregivers on exercises, mobility, and safety techniques
  • Maintain accurate and timely clinical documentation in compliance with regulations
  • Collaborate with physicians, nurses, and interdisciplinary care teams
  • Ensure compliance with Medicare and home health guidelines

Required Qualifications:

  • Degree in Physical Therapy (PT)
  • Active Physical Therapist license (or eligibility) in the state of Pennsylvania
  • Strong clinical and patient care skills
  • Ability to work independently and manage a flexible schedule
  • Excellent communication and organizational skills
Preferred Qualifications
  • Prior home health experience
  • Familiarity with OASIS documentation
  • Experience with EMR systems (e.g., HCHB)
