Job Title: Home Health Physical Therapist
Coverage Location:
The Villages and surrounding areas
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Job Type: Local contract
Contract Length: 3+ months
Pay Range:  $52-$56/hrs 
*Pay offered is based on experience, expertise, credentialing, and education.  

Weekly Hour Guarantee: 40 hours
On-Call/Weekend Requirement: Yes
Mileage Reimbursement: Yes (between patient visits) Key Responsibilities
  • Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks
  • Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence
  • Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education
  • Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques
  • Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems
  • Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides
  • Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team
  • Participate in case conferences, care coordination activities, and team collaboration
  • Ensure compliance with CMS regulations, infection control standards, and agency policies
  • Manage a mobile caseload efficiently while meeting productivity and documentation expectations
  • Adapt quickly to new environments, workflows, and patient populations as a travel clinician
Required Qualification/Certification
  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapist license in the state of assignment (Florida, if applicable).
  • Minimum of 1 year of clinical experience as a Physical Therapist (home health preferred)
  • Experience treating older adults and medically complex patients
  • Strong ability to work independently with sound clinical judgment
  • Proficiency in electronic documentation systems and mobile technology
  • Valid driver’s license, reliable transportation, and active auto insurance
  • Willingness to travel within the assigned coverage area
  • Active Physical Therapist license in the state of assignment
  • Current BLS certification
  • Valid driver’s license.
