Job Type: Contract
Contract Length: 3 months (possible extension or conversion)
Pay Range: $38/HR- $42/HR
*Pay offered is based on experience, expertise, credentialing, and education.

Duties:
The Physical Therapist Assistant (PTA) provides skilled physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist (PT). The PTA implements the established Plan of Care, documents patient response to treatment, and communicates patient status changes to the interdisciplinary care team in accordance with agency policy, state regulations, and payer guidelines.
Requirements:
- Home Health experience required.
- HCHB is required.
- Strong experience with OASIS documentation.
- Certification(s): Active Physical Therapist Assistant license (state-specific)
Additional Requirements:
- Ability to travel daily to patient homes within assigned territory (50 miles radius).
- Valid driver’s license and reliable transportation.
- Comfortable working in home-based care environments.
