Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician referrals.
  • Complete comprehensive evaluations using appropriate assessments and tools.
  • Review patient records, including medical history and precautions.
  • Establish measurable goals and develop treatment plans for patient progress.
  • Regularly reassess treatment effectiveness and patient progress towards goals.
  • Communicate patient needs and progress to the treatment team and relevant parties.
  • Consult with medical providers regarding patient evaluations and treatments.
  • Recommend and assist in securing adaptive equipment for patients.
  • Maintain and submit documentation as required by regulations.
  • Supervise Physical Therapist Assistants and Aides.
  • Participate in staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist
  • WV PT license
  • Home Health experience with OASIS preferred
  • Experience with HCHB preferred
