Job Title: Physical Therapist Profession: Physical Therapist Specialty: - Duration: 13 weeks Shift: M-F Hours per Shift: 8:00 AM - 4:30 PM Experience: Five years of progressively responsible PT experience. License: Valid Physical Therapy License Certifications: BLS Must-Have: - Minimum of two years of Physical Therapy experience in a clinical setting with a frail or elderly population. - Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. - Two years supervisory experience with demonstrated ability to mentor and develop team members. - Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members. Description: The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants. The role includes working in close coordination with the Physician and rehabilitation staff to maximize participant independence and safety. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department. The Physical Therapist is a member of the management team and facilitates collaborative relationships to promote values, team culture, and mission. Key duties include conducting Physical Therapy assessments to determine care needs and goals. The Physical Therapist will participate in interdisciplinary meetings to develop participant care plans. The role involves delivering and documenting interventions in accordance with physician orders and care plans. This includes providing active and passive exercise, muscle re-education, and functional training of activities of daily living. Supervision of therapy assistants is required, along with providing training, instruction, and guidance for Functional Maintenance Programs. Training and instructions will also be provided to staff, participants, and caregivers in the use and care of therapeutic equipment. The Physical Therapist will manage equipment orders and maintenance while remaining alert to input from team members and caregivers. The role requires supporting smooth care transitions between different settings. Participation in enrollment efforts for prospective participants into the program is also expected. End-of-life care coordination and support may be part of the duties. The Physical Therapist is responsible for managing direct reports and completing all required documentation in a timely manner. Privacy and confidentiality of all procedures and participant information must be protected. Following policies and procedures and participating in required Quality Improvement activities is necessary. Regular communication with supervisors and team members regarding workload and priorities is essential. Flexibility in performing assignments is required as business needs evolve. Other duties may be assigned as necessary.
