Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Deliver skilled physical therapy interventions to restore and maximise functional mobility
- Monitor patient progress and adjust treatment plans as clinically indicated
- Document evaluations, progress notes, and discharge summaries per regulatory and facility requirements
- Collaborate with the interdisciplinary care team to coordinate continuity of care
- Educate patients and caregivers on home programmes and safe mobility techniques
Requirements
Must-Have
- Active state Physical Therapist (PT) licence current and in good standing
- CPR/BLS certification current
- Graduate of an accredited physical therapy programme
Preferred / Nice-to-Have
- Experience in a skilled nursing or rehabilitation setting
