Job Overview
We are seeking a licensed Physical Therapist (PT) for a travel contract assignment in a skilled nursing and rehabilitation setting. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions to improve mobility, strength, and functional independence.
This role is ideal for PTs who enjoy working in a fast-paced rehabilitation environment with interdisciplinary care teams.
Key Responsibilities
Evaluate patients and develop individualized treatment plans
Provide hands-on physical therapy interventions
Monitor patient progress and adjust treatment plans as needed
Educate patients and families on exercises and recovery plans
Collaborate with nursing and rehab teams
Maintain accurate clinical documentation
Support patient mobility, strength, and functional improvement
Ensure compliance with clinical standards and facility protocols

Requirements
Active Physical Therapist (PT) license required
Degree in Physical Therapy from accredited program
Previous rehab or skilled nursing experience preferred
Strong clinical assessment and treatment planning skills
Ability to work independently and in multidisciplinary teams
CPR/BLS certification preferred
Must comply with onboarding and credentialing requirements
Must bring state or federally issued identification and wear a photo badge at all times
