Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluation and treatment based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools
  • Review patient records for diagnosis, history, and necessary precautions
  • Establish measurable goals and develop personalized treatment plans
  • Reassess treatment effectiveness and patient progress regularly
  • Communicate patient needs and progress with the treatment team and family
  • Consult with medical providers about patient evaluations and progress
  • Recommend and assist with adaptive equipment for enhanced patient function
  • Maintain and submit necessary documentation per required standards
  • Supervise Physical Therapist Assistants and Aides
  • Participate in staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist
  • Licensure in Physical Therapy
  • Proficiency with Oasis and HCHB software
  • Experience in home health is a plus
