Description: The Physical Therapist partners with residents and other care providers to provide physical therapy services to maximize residents' functional abilities and independence in their activities of daily living. The therapist performs evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider while observing precautions, indications, and contraindications. Evaluates residents per department protocols and establishes a therapeutic plan of care that includes functional long- and short-term goals, frequency and duration of services, and therapeutic modalities to be used. Seeks input from the resident and primary caregivers regarding the therapeutic plan of care and incorporates this information into the therapy goals. Provides consultation to the referring physician and other members of the healthcare team regarding the plan of care and discharge needs of the resident. Attends all pertinent team meetings. Monitors and evaluates the treatment effect and progresses the treatment program toward the functional goals established in the treatment plan of care. Modifies the treatment plan of care as necessary. Documents in the Electronic Medical Record all evaluative and therapeutic interactions with the resident per department procedures. Ensures that the documentation is accessible to others on the care team, including ensuring language that can be easily understood. Recommends and obtains appropriate assistive devices for the resident and ensures that instruction has been given in their use. Provides appropriate education to the resident and/or caregiver and documents the results of teaching. Refers residents appropriately to Physical Therapist Assistants. Works collaboratively to ensure good resident care with other therapy professionals. Understands and complies with Medicare and other reimbursement regulations. Documents resident care appropriately with attention to Medicare, Medicaid, Private Pay, and other reimbursement requirements. Provides in-service education regarding clinical knowledge and skills to nursing and other departments. Maintains current clinical skills and knowledge through learning activities. Assists with the development of new programs and services offered from Rehabilitation. Maintains awareness of professional issues through membership in professional organizations and reading professional publications. Brings pertinent matters to the attention of others on staff. Supervises the activities of the assistant-level staff when appropriate, considering their level of education and training. Ensures adherence to all state mandates and professional organizations' standards for using assistant-level staff. Brings performance issues to the attention of the Director. Provides mentorship to new staff during orientation, ensuring that the department orientation checklist and basic competencies are completed per departmental procedure. Ensures department cleanliness and maintenance by routinely keeping office and clinical areas clean and neat. Assists with routine equipment and space cleaning. Follows all system and department infection control procedures. Maintains required licenses, certifications, and competencies and completes annual compliance courses on time. Performs other duties as consistent with this role. Must possess the physical and mental abilities to perform tasks normally associated with this position, which involve a combination of sitting, standing, walking, bending, stooping, twisting, and reaching. Must be able to lift up to 50 pounds. Above 50 pounds requires a 2-person lift. Lifting/carrying frequently up to 20 pounds. Some stress is related to a high level of responsibility for quality care.
