Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Days Hours per Shift: 8:00 AM - 5:00 PM (Schedule can be flexible based on patient's availability) Experience: Minimum 1 year of Home Health experience License: Valid Physical Therapy license Certifications: CPR certification preferred Must-Have: One year of Home Health experience Description: The primary goal of the Physical Therapist is to restore function and minimize disability following disease or injury. The therapist helps clients reach their maximum performance/function within their capabilities. The role involves evaluating and treating patients using medically prescribed physical therapy programs. Supervision of Physical Therapy Assistants may be required. Initial admission and evaluation visits will be conducted by the therapist. The individual will notify the assigned Clinical Team Manager of relevant scores within established timelines. Documentation of observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans is essential. Other duties may be assigned in alignment with the goals and mission of the organization. Territory may include neighboring cities as needed. Holiday coverage may be required.
