Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 26 weeks Shift: Days Hours per Shift: 8am-5:30pm Experience: 1 year of Home Health experience License: Valid Physical Therapist license Certifications: N/A Must-Have: Home Health experience Description: The primary goal of the Physical Therapist is to restore function to prevent or minimize disability following disease or injury. The therapist helps clients reach their maximum performance or function within the limits of their capabilities. They will evaluate and treat patients using medically prescribed physical therapy programs. The Physical Therapist may supervise Physical Therapy Assistants, make initial admission/evaluation visits, and notify the assigned Clinical Team Manager of OASIS scores within established timelines. The ideal candidate will document observations, clinical findings, problems, skilled interventions or treatments, goals, and discharge plans. This individual will also perform other duties as assigned consistent with their skills and training in alignment with the organization's goals and mission.
