Job Title: Physical Therapist - Home Health Profession: Physical Therapist Specialty: Home Health Duration: 16 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: 1 year License: Physical Therapist Professional license in Massachusetts Certifications: None specified Must-Have: 1 year home care experience Description: Provide physical therapy services to patients in their place of residence. Patients may have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients can range in age from birth to over 100 years. Collaborate with the patient, family, physicians, and other allied health professionals. Identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications. Follow policies, procedures, and standards of practice. Weekend rotations are required once every 6 weeks. Ability to travel to assigned cases is necessary. Start date based on credentialing clearance. If not cleared by posted start date, a rescheduled start date will be necessary.
