Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 1 year of skilled home health experience License: Pending license accepted Certifications: BCLS required Must-Have: Oasis experience required Description: Home Health Physical Therapist position available. Candidates will be expected to provide therapy services in patients' homes. The schedule is 8-hour days, and the facility cannot accommodate alternate shift options. Therapists may visit up to 6 patients per day, with a standard productivity expectation. The facility can call off up to one shift per pay period. Travelers are expected to have their own transportation for patient visits. Drive and windshield time are minimized, but some travel of up to 50 miles may occur. Primary series COVID vaccination and one booster are required. Candidates must be familiar with Oasis for patient documentation. Clothing requirements allow for any color scrubs. The ideal candidate will demonstrate the ability to provide quality care and manage a diverse caseload. References are required from previous supervisory roles within a comparable specialty. Candidates should be comfortable working independently in a home health environment. Flexibility and the ability to adapt to patient needs are essential. All travelers are expected to float between facilities within the state as necessary. Refusal to float may lead to assignment termination.
